Lough Corrib navigation marks

There are some interesting comments about Lough Corrib’s navigation marking system in this report [PDF] by the Marine Casualty Investigation Board.

Float Bridge

This post was originally entitled “The avarice of the ferryman” but, as more information has been added, it seemed best to name the post for the remarkable Float Bridge itself, which links road, rail and waterway transport.

The avarice of the ferryman

Castlepollard, Sept 11. Last Week the following Accident happened at the Ferry, or Float, plying for Passengers over the River Inny, in the County of Westmeath: — A Post-Chaise and Four, with a Lady and Gentleman, were imprudently put upon this dangerous Conveyance, without separating the Cattle from the Carriage; unfortunately a Car and Horse had been put in before them, which, with the Post-Chaise and Horses, occupied the full Length of the Float.

On the Passage, the Car Horse grew very uneasy, and going back, the Car annoyed the Post-Chaise Horses, which occasioned them to back in like Manner, until the Post-Chaise fell into the River, and dragged the Horses after it; three of the Horses were drowned, being entangled with the Harness; the other broke through his Harness, and swam over to a boggy Place, but could not get upon Land; one of the men followed him in a small Boat, to lead him to a proper landing Place, but not being able alone to guide the Horse and row the Boat, the Horse got too near it, and striking it with one of his Feet, overset and sank it, by which the Man was drowned; the Horse then swam, and was saved.

It was very lucky for the Lady and Gentleman that they alighted from the Chaise at going into the Float. The Carriage, which belonged to the Gentleman, was got out with much difficulty; the Horses were Hacks. The Avarice of the Ferryman occasioned this melancholy Accident.

Good shot wanted

FERRY-BRIDGE, over the River Inny, between the County of Westmeath and Longford, 3 miles from Castlepollard, 12 miles from Mullingar and Longford, Sept 1st, 1774. Complaint having been made, that the Smallness of the Float rendered it inconvenient, and occasioned timorous People to drive or ride many Miles round to avoid the Ferry, the Proprietor therefore has undertaken to build a Bridge at his own Expence, which will be finished with all convenient Speed; in the mean Time, a Part of the intended Bridge, above 30 Feet long, Battlements fixed on each Side, properly gravelled over &c, will be made Use of to ferry over Carriages, &c. A Coach and four may now pass with the utmost Safety, without taking off the Horses, or 20 Head of Cattle, &c in less than 2 Minutes; and, to accommodate Graziers and others, as soon as said Bridge is compleated, Droves of large Cattle, above 30 in Number, will be passed over at the Rate of a British Shilling per Score, private Soldiers with Furlows from their Commanding Officers, in Time, gratis, all other Passengers, Cattle, &c at the usual Rates taken above these 20 Years.

This Road is now in good Repair, and well known to be many Miles nearer from Dublin to the County of Longford, Leitrim, Roscommon, Sligo, Mayo, &c than any other Road; a commodious Inn, near Ferry-bridge, on the Westmeath Side, is building, and a Carriers Inn on the Longford Side, will be both soon finished, and proper People to keep them.

Wanted, to take Care of said Bridge when finished, and to collect the Toll, &c a sober, honest, careful, active, middle-aged, single Man; he must be a Protestant, write a good Hand, and if a good Shot, and understands fishing in Lakes and Rivers, and delights in those Amusements, it will be more agreeable &c. Comfortable Lodging and Board, and not less than £12 per Ann will be made good to the Person approved of, and shall be treated (as far as can be reasonably expected) agreeable to his former Manner of Life. None need apply but such as have an undeniable good Character, as to Honesty and Sobriety, from his former Employers or Neighbours. Inquire of the Printer hereof.

Sir Thomas Chapman

The Dublin Evening Post of 9 August 1810 advertised part of the Meath and Westmeath estates of Sir Thomas Chapman Bart to be let. They included

The Tolls of the Float near Castlepollard
And an excellent house and fifty acres of land

Applications were to be sent to Sir Thomas at St Lucy’s, Athboy, or to Mr High Dickison at the same address.

St Lucy’s was also known as Killua Castle, set of the Chapman baronets, of whom Sir Thomas was the second. Do be sure to read about the seventh baronet.

Sir Benjamin Chapman

Sir Benjamin James Chapman was the fourth baronet. Ewan Duffy writes:

Float bridge was a privately owned toll bridge. Its owner, Sir Benjamin Chapman, offered the bridge to the Midland Great Western Railway if they would build a station at Float, for which he would also give the necessary land. He subsequently suggested a variation on the agreement that if the company were to cease using the station, the land and bridge should be re-conveyed to him!* As the bridge remained in CIE ownership up to 1971, when it was transferred to Westmeath County Council under the Transport (Miscellaneous Provisions) Act 1971, he clearly did not get such a deal!

The original bridge is no longer there — I paid a site visit there last year, given its railway connection, but it has been replaced sometime in the 20th Century.

Boater’s handbook

The Canal & River Trust have a useful PDF Boater’s handbook available for download, free of charge, from this page; there is also a video thingie.

The handbook naturally concentrates on British inland waterways, with narrowboats on canals and rivers but not on lakes, but there is nonetheless much useful material. Even experienced boaters might find it of interest, if only for briefing any non-boaty friends they’ve invited along for a trip.

 

DUKWs? Fiat lux

I have written here about a series of misfortunes suffered by DUKWs in Liverpool and London. On 16 June 2013 I put up some photos of DUKWs in Dublin and Liverpool; I pointed to what seemed to me to be two differences between practices in the two cities:

First, before they enter the water at Grand Canal Dock, Ringsend, the DUKWs are fitted with extra buoyancy in cylinders that slide into racks along their sides. I saw the VikingSplash crew removing the cylinders from the yellow DUKW; it took only a couple of minutes, and I presume that it didn’t take much longer to put the cylinders on.

Second, the Dublin passengers are issued with buoyancy aids before they take to the water. I can’t see any buoyancy aids on the Liverpool passengers, although it’s possible that they are out of camera shot.

There are links on that page to photos, news reports and a seriously scary video of the sinking of a DUKW in Liverpool. Then, in September 2013, a DUKW went on fire on the Thames; my brief report and links here. In October 2013 the UK Marine Accident Investigation Branch issued a safety bulletin (my report here, with links) pointing to foam buoyancy as a common factor. The Liverpool DUKWs did not have enough buoyancy to keep them afloat if they started taking water and MAIB thought it would be impossible to get enough into them. On the London vessel,

… the most likely cause of fire was the action of the rotating drive shaft (or other moving parts) on the oil contaminated surfaces of the buoyancy foam blocks.

In November 2013 I noted that the wearing of lifejackets had been discussed in London and I commented on the policy of the Dublin operator, VikingSplash:

The point that strikes me is that, in both UK accidents, passengers had little time to don lifejackets and would have been trying to put them on in a confined space and under less than ideal conditions. It seems to me that Viking Splash’s policy [having passengers don lifejackets before taking to the water] is the right one.

In December 2014 the MAIB published its report into the two accidents. There’s a Guardian news report here [h/t gjb] and you can download the MAIB’s full report and annexes from this page. The London and Liverpool accidents are covered in the same report.

It’s well worth reading and pulls (as far as I could tell) no punches, even tearing strips off the Maritime and Coastguard Agency and the Vehicle and Operator Services Agency, both within the UK Department for Transport and both involved because the DUKW is an amphibian.

I took three main points from the report.

The Irish approach

First, the Irish authorities seem to have thought seriously about the safety problems. My understanding of the buoyancy requirements was mistaken: the UK DUKWs had added buoyancy (although not enough, and adding more foam caused fire) and the Irish buoyancy cylinders are not designed to keep the vessel afloat. Here’s what MAIB says:

In Ireland, APV operators have been permitted to operate vintage DUKWs without having to provide any residual buoyancy. To mitigate the consequences of serious flooding, the Irish regulator required the operators to:

  • Fit external buoyancy tubes designed to slow the sinking process and make the vehicle sink bodily [my emphasis].
  • Retract the canopy roof and open the side curtains prior to entering the water.
  • Require passengers and crew to wear PFDs while on the water.
  • Provide a fast rescue craft, rescue crew and an inflatable liferaft at the slipway.
  • Limit operations to a non-tidal area.

This approach focused on passenger survivability by reducing the risk of entrapment and drowning, rather than vehicle survivability, and introduced several of the interim measures recommended by the NTSB following the sinking of Miss Majestic.

The Irish model demonstrates that open topped APVs can be operated successfully in similar weather conditions to those experienced in the UK, and that passengers are willing to wear PFDs.

So big it up for the Irish Maritime Administration.

The speed of the sinking

Second, if you’ve seen the video of the Liverpool sinking you may have been struck by its speed. In both Liverpool and London passengers had very little time to get out and the report’s synopsis says

In both instances, the crew had little time to co-ordinate the evacuation process and the confined nature of passenger spaces made it almost impossible for them to control or assist the passengers.

And in 4.3 Common safety issues:

8.  It was extremely fortunate that all on board WQ1 and Cleopatra were able to evacuate into the water unharmed. In both cases the passengers were forced to act on instinct and exit the vehicles under their own initiative.

Any of several issues could have cut the time available and “the risk of entrapment and the likelihood of loss of life would have been considerably higher”.

So Figure 63 of a Dublin DUKW doesn’t just show the external buoyancy cylinders: the passengers are wearing buoyancy aids, there is a crewman already stationed at the stern and the side and roof canopies are open, all giving more chance of escape.

I’m not in any position to assess the overall safety of the Irish DUKW operation. What interests me here is a more general point about the evacuation of passengers from trip vessels: getting a large number of people out of a small space in a short time is not easy. And the recent problem of getting people off the Norman Atlantic didn’t make me feel any better.

Third, I am delighted that my photo of a DUKW in Liverpool was useful to the  UK Marine Accident Investigation Branch (see the report’s Figure 16) but they really should have asked for permission to use it.

 

Meath River Rescue

I have no idea what this Dáil written question and answer, from 12 November 2013, are about:

Peadar Tóibín [SF, Meath West]: To ask the Minister for Environment, Community and Local Government if his Department will provide direction for a safety procedure (details supplied) to be enforced; if this procedure been enforced in any other region or with any other community organisation; if he will reverse this decision and allow for Meath River Rescue to operate in a fully integrated and efficient manner.

Phil Hogan [FG, Carlow-Kilkenny; Minister for the Environment, Community and Local Government]: My colleague, the Minister for Transport, Tourism and Sport is the Minister with responsibility for receiving calls for assistance for inland waterways and mobilisation of appropriate response services. I have no function in the matter referred to in this question.

Googling suggests that Meath River Rescue got €278,358 for a boathouse and training room in Navan, with two FG TDs announcing the grant a year apart [or is one of those dates a typo?]. The service is known to Irish Water Safety but is not one of its Community Rescue Boats Ireland so it’s not a Coast Guard declared resource [which doesn’t mean there’s anything wrong with it]. Meath River Rescue’s own website and facebook page don’t seem to have been updated since April 2012 [although I am unfamiliar with facebook and may not be interpreting the dates correctly] but the service has been in action as recently as September 2013.

Unfortunately “details supplied” means supplied to the minister, not to readers of Dáil proceedings, so I have no idea what problem was being raised.

 

 

 

DUKWs and lifejackets

On 31 October 2013 I mentioned the UK Marine Accident Investigation Branch’s safety bulletin about the DUKW fire in London and the DUKW that sank in Liverpool. There is more on the London fire today with a Guardian report on proceedings at the London assembly’s Thames passenger boat investigation committee. The Guardian headline read …

Duck boat passengers not wearing lifejackets when jumping into Thames

… and the story reported the Maritime Coastguard Agency’s maritime safety and standards director as saying that wearing of lifejackets would not have been usual on “such boats” and that lifejackets were safely stowed above the seats. The story also said that

London Duck Tours’ managing director, John Bigos, said the Cleopatra had the required legal number of lifejackets on board and that it was company policy that lifejackets were not worn on tours. He went on: “We have our reasons for this (non-wearing) but they are not to do with commerciality.”

There is a different policy in Ireland, where the Dublin Viking Splash operation says

Lifejackets: At the water entry point, customers are required to put on a lifejacket after the driver delivers an outline about safety on the water. The lifejackets supplied by Viking Splash Tours are Solas and CE approved buoyancy aids […].

The point that strikes me is that, in both UK accidents, passengers had little time to don lifejackets and would have been trying to put them on in a confined space and under less than ideal conditions. It seems to me that Viking Splash’s policy is the right one.

DUKWs

In June 2013 I reported on the sinking of a DUKW in Liverpool, the second of the year, and in September on the fire aboard a DUKW in London. Oddly enough, it seems that the two accidents had a common element: the foam added inside the hulls to improve buoyancy.

The UK’s Marine Accident Investigation Branch has found that the DUKWs in the Liverpool fleet had not been fitted with enough foam to provide 110% buoyancy, enough to keep the vessel afloat when flooded. MAIB’s tests on the Liverpool vessels

… raised serious questions about whether the operators of DUKWs could fit sufficient foam internally to comply with the current requirement for 110% buoyancy without compromising the safe operation and the practical day to day maintenance of these vehicles.

The “safe operation” part affected the London vessels. In July 2013, a DUKW had to be towed in after a drive shaft universal coupling failed when it overheated and ran dry of lubrication: engine bay temperatures were too high because of the volume of foam inserted. And a report commissioned by the London Fire Brigade into the September fire concluded that:

… the most likely cause of fire was the action of the rotating drive shaft (or other moving parts) on the oil contaminated surfaces of the buoyancy foam blocks.

MAIB has issued a safety bulletin [four-page PDF] with this conclusion:

The MAIB identified significant difficulties in fitting a DUKW with the volume of foam required to meet the buoyancy standards set out in MSN 1699 (M). Further, the nature of these old amphibious vessels, specifically their weight in relation to their size and the complexity of their propulsion arrangements, makes it difficult for operators to comply with the standards applicable to more conventional craft by solely using internal foam buoyancy. An alternative standard, ensuring that DUKWs have the necessary level of damage survivability, therefore needs to be established if they are to be operated safely.

It has recommended that the Maritime and Coastguard Agency should

… ensure that the means used by DUKW operators to achieve the required standard of buoyancy and stability for their vessels does not adversely impact on their safe operation. Furthermore, these vessels should not be permitted to operate until satisfactory levels of safety can be assured under all feasible operating conditions.

The Maritime and Coastguard Agency’s website doesn’t seem to be working, but there is a BBC report here quoting an MCA person as saying

We will not be permitting the vessels to operate until we are satisfied that the necessary safety measures have been achieved.

All of this may help to explain why the VikingSplash DUKWs operating in Dublin are fitted with additional external buoyancy.

London DUKW

A few months ago I wrote about the sinking of a DUKW trip boat in Liverpool. Today (h/t FisherBelfast’s Blog) comes news of a DUKW on fire on the Thames. Happily, nobody died and there were no serious injuries.

Waterproof wireless telegraphy

A couple of weeks ago I wrote about wireless telegraphy, concluding that:

At present, the rational decision for a boat-owner is to buy a cheap VHF without bothering to get either a certificate for the operator or a licence for the vessel. This is the rational decision because the official channels for getting certificates and licences are slow, expensive and cumbersome. It may therefore be — who knows? — that the populace has already decided to ignore the regulations.

For leisure boating within some sensible distance of the shore, I suggest that the current regulations be either drastically simplified or, perhaps better, scrapped altogether. That might mean giving the International Telecommunication Union a kick up the transom, but the present system is counterproductive: it seems to limit the use of handheld VHFs in cases where they could be very useful, if only to allow search and rescue volunteers to stand down earlier.

Two reports published today by the Marine Casualty Investigation Board have caused me to reconsider that conclusion.

In the Brownstown Head case [Report no MCIB/229; no 11 of 2013] [PDF], two occupants of a 16′ punt were thrown into the water when the boat capsized. They had a handheld VHF in the boat, but it sank when the boat capsized; one occupant put his mobile phone in his mouth to keep it above the water but it was knocked out. As a result, they were unable to summon assisstance. They eventually tried to swim to the shore, but only one made it; the other was drowned.

In the MacDara’s Island Currach case [Report no MCIB/215; no 10 of 2013] [PDF], a fisherman seems to have fallen overboard from a 6.15m open boat, in which he was alone. It was equipped with flares and a VHF in a watertight container, but as the report says:

5.6. There are unique problems with fishing operations from small open boats by lone fishermen. Once they become separated from their vessel their means of communicating their distress are on board their boat and not accessible to them.

In neither of these cases was the possession of a handheld VHF of any use. The VHF sets would have had to be (a) waterproof and (b) securely attached to the boaters’ lifejackets. The report on the MacDara’s Island Currach case recommends:

6.5. That the “Code of Practice for Fishing Vessels under 15 LOA” section 9.5 Radio Equipment should be amended by the addition of a requirement for undecked vessels where there is a lone occupant that an appropriate beacon should be of the type worn on the person.

In fact, the same recommendation could be applied to cases like Brownstown Head, where there were two people in the boat. That is broadly in line with the actions announced by two ministers on 8 July; note Simon Coveney also said:

I am also establishing a new high level working group on safety in the fishing industry, to look at all aspects of safety on fishing vessels and to report to Minister Varadkar and myself with recommendations before the end of the year. The new working group will be chaired by Mr John Leech current CEO of Irish Water Safety. Because a common thread of comment in recent times has been the need to pay particular attention on issues surrounding the number of small inshore boats that get into difficulty, I have charged the group with focussing to a large degree on this aspect.

So the wider use of handheld VHFs won’t solve every problem. I still think, though, that such wider use should be encouraged by the removal of unnecessary barriers.
